The SAFESTART
Accident/Incident Investigation Enhancement Series
is a reality-based training program—a series
for enhancing accident and incident investigations.
The enhancement comes from incorporating the
SAFESTART principles into the investigation
process. By looking at the states and errors
that are involved in most accidents and injuries,
this program will shed new light on root causes
(human factors) and provide a whole new perspective
on accidents in your organization.
The
SAFESTART Accident Investigation program will
teach and motivate employees to conduct their
own investigations, on the close calls and
small errors they make, so that big incidents are
avoided (SAFESTART Critical Error Reduction
Technique or CERT #2). Formal accident/incident
investigations are usually only conducted when a
serious incident occurs. With the SAFESTART
Accident Incident program, an organization benefits
by having people learn from the hundreds of minor
incidents that occur every day.
This program will improve safety on two levels:
Level One:
When a site trains their
investigators with SAFESTART Accident
Investigation these employees are:

trained on the
benefits of looking at the states and errors
involved in their own minor incidents, and
to think about which CERT technique might
have prevented it.

they will now
ask these questions with other employees
when they are doing their formal incident
investigations, and may possibly discuss
these techniques with others even for small
incidents they see/hear about. |

as more of the
workforce are trained in AI, and as more
investigations are conducted using the
SAFESTART concepts, the benefits of
conducting your own
“states/errors/techniques” review even for
minor incidents will become more widespread—part of the culture. |
Level Two:
If the client site also
conducts Employee Overview sessions that reinforce
the value of “CERT Technique #2”, they can reinforce
the original SAFESTART training on this
technique and expedite the widespread adoption of
“personal investigations” for minor incidents.

It is REAL –
real people, real accidents – so this adds
to the credibility of the training
experience (not just theory and actors). |

It is
ENHANCEMENT not replacement. What you are
already doing for accident investigation is
good. However, if we just keep doing what
we’ve always done, we won’t improve our
results from what they’ve always been. |

The focus is to
prevent SIMILAR incidents, not just the same
incident, from occurring again. Often
accident investigation results in an
engineering or procedural “fix” that will
prevent the exact same incident from
occurring again, but won’t necessarily
prevent similar incidents. For example, we
need to help people get better at “moving
their eyes before they move their bodies”,
in addition to creating a better shelving
system for jagged metal. |

on HELP instead of blame.
